  • Use the money for any purpose, medical or non-medical.

Plan Rates - Hospital Indemnity

  • Employee Class: Regular Full-Time Eligible Employees

  • Employer pays: 100% for A $50,000 Life and AD&D Policy and 100% for the STD and LTD

  • Employee pays: 100% for Voluntary Life, Hospital Indemnity, Accident, and Critical Illness if the elect

  • Waiting Period: 1st of the month following 30 days from date of hire
